Frequently asked questions
You will receive an email or text message from our Access Center, which will include a link to your pre-appointment questionnaire. If you do not have electronic access, make sure to arrive at least 30 minutes early to your appointment to find parking, find your check-in floor, and to fill out your questionnaire before you are seen. The questionnaire will include questions about your health, your health history and your care goals. Some of these questions may be sensitive, but they help your care team determine the right care plan for you.
Completing the questionnaire at home allows your care team to create an individualized care plan, before your first visit, to help you reach your goals faster.
Please arrive at least 30 minutes before your appointment. If you have not completed your pre-appointment questionnaire (see "What happens before my appointment?" above), you will be asked to do so at check-in. You can complete the questionnaire privately or our trained concierge staff can help answer them with you.
