The UT Medicine patient portal, powered by MyChart, gives you safe, convenient access to parts of your UT Medicine health record — anytime, anywhere. 

With your UT Medicine patient portal account, you can:

  • View upcoming appointments 
  • View summaries from previous visits.
  • Book new appointments 
  • Request prescription refills.
  • Securely message your care team, with the ability to include attachments via mobile web browser.
  • Access test results, immunization records, billing information and patient education materials.

For patients with existing MyUTHA patient portal login credentials: You will need to sign up for a new account with the UT Medicine patient portal. Your MyUTHA credentials will not work in the new patient portal.

Digital check-in

A centralized, digital check-in area for patients is now located on the second floor of the Health Transformation Building. Kiosks are designed to make your check-in experience efficient and seamless. If you get stuck, don’t worry. One of our friendly ambassadors will be available to assist you. 

If you already know where your appointment will take place, you may also check in at the kiosk on your clinic floor. Digital check-in will also be available through the new patient portal and app, saving you time ahead of your visit.

Yes. We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords. You control your password.

No. The UT Medicine patient portal is not monitored 24/7 and should not be used for urgent or life-threatening concerns. Please call 911 in case of emergency. 

Absolutely not. You’ll continue to receive the same exceptional, personalized, whole‑person care from the team and the clinic you’ve come to trust.

All messages are reviewed during normal business hours. Responses may take up to three business days.  

Yes, with approved proxy access. Proxy access allows you to view another person’s UT Medicine patient portal account (such as a child, spouse or dependent). 

After logging into the portal, you can complete, sign and submit the proxy access request and authorization form in MyChart. You may also download the form or request one at the Welcome Center on the second floor of the Health Transformation Building. Adult patients 18 years of age and older can grant access to their records directly from the UT Medicine patient portal using the proxy invite feature.   

Proxies have full access to a child’s chart until the child turns 18 years old. Beginning at age 13 and with parental/legal guardian consent, patients can have their own portal account.

No. You do not have to be a patient to be granted proxy access. 

Most test results are released automatically as soon as they are available. It’s possible you will see your results before your provider has had a chance to review them.  

The timing of when results appear in the portal depends on the type of test and your notification preferences. You can review and adjust your preferences in the portal to choose how and when you’re notified about new results.  

Yes. Digital check-in is a feature of the UT Medicine patient portal that allows you to complete registration and required pre‑visit tasks from home. It’s a convenient timesaver that reduces time spent at the front desk and shortens wait times on arrival. 

Telehealth video visits and eVisits are convenient ways to receive care without coming into a clinic. 

A video visit allows you to meet with your provider in real time using video through the UT Medicine patient portal. This is similar to an in‑person appointment but conducted virtually. Choose this option if you want to talk with your provider face‑to‑face, ask questions, or discuss a new or more complex concern.  

An eVisit is a secure, online message‑based visit where you answer questions about your symptoms through the patient portal, and a provider reviews your information and responds. An eVisit is ideal if you have a non‑urgent issue that can be addressed through questions and secure messages. 

Patient‑reported outcomes are questionnaires that ask you to share information about your symptoms, how you’re feeling or how your health affects your daily life. Your responses help your provider prepare for your visit, track changes over time and make more informed decisions about your care.

Patient-reported outcomes do not replace conversations with your provider, but they help ensure your concerns are clearly communicated and addressed. 

Most providers offer messaging through the portal, but some may not have messaging enabled. You may also be unable to send a message if you have not had a recent visit with that provider.

Many visits can be scheduled online, but some require a call to the clinic due to the nature of the visit.  

Click “Forgot Password?” on the UT Medicine patient portal log-in page and follow the instructions. You will need access to the email address or mobile number on file.

Yes. Two-factor authentication is an industry standard security measure that helps protect patients’ accounts and information.
